Description for case is given below :
In mid-April 2010, Clark McCullough, a partner at Ardent Capital, reviewed the final investment memorandum concerning a possible $110 million investment in AtHomeCare, Inc., a private company providing home health care services. Over the course of the previous year, Ardent Capital had completed preliminary due diligence, and in the fall of 2009, it had signed a letter of intent (LOI) and had been granted an exclusivity agreement to consider a potential purchase of the company. Although the company fit well within Ardent’s current areas of investment focus, the deal had been conceived as a rollup strategy, in which AtHomeCare would serve as an investment platform, and other health care services companies would be acquired to build a larger entity. A large portion of the due diligence had focused on finding a suitable acquisition target, but to date no target had been locked in. With the LOI agreement set to expire later in the month, the firm’s investment committee would now have to decide whether to proceed with the purchase of AtHomeCare on a stand-alone basis with only the prospects of yet-to-be-determined acquisitions or delay the purchase until an add-on acquisition surfaced.

This case address two major issues of importance to an integrated delivery system ( IDS) in promoting goal congruence: the design of financial responsibility centers and the selection of an appropriate transfer pricing methodology. It helps students to see that, in a truly integrated IDS, treating hospitals (and other providers) as standard expense centers and using two-part transfer prices can help to improve goal congruence.

This case is useful in bringing together several different disciplines: strategy, management control, and change management. The organization was a newly formed integrated clinical enterprise that included an academic medical center, a community hospital, and a academic practice plan. It also was tied strategically and financially to a college of medicine. It was attempting to achieve clinical and financial integration between its physician practices and its provider institutions, mainly the medical center, but the effort had been compromised by a budget crisis that had had a detrimental effect on the organization, and had led to mistrust among the physicians, some of whom were considering leaving the organization.

The physician leaders need to decide how to change the system, given that a major payer, representing about 30% of its revenue, was switching from fee-for-service to capitation.

Description for case is given below :
A very comprehensive case that requires students to define the organization’s strategy, understand its structure, and then determine how a responsibility center network has been overlaid on it. There is a poor programming process at work, which has led the organization in some unusual directions, a budgeting process that emphasizes revenues and ignores expense, and a reporting process that does not fit well with the organizational structure or the network of responsibility centers. In short, a mess.

Description for case is given below :
This case introduces Canada’s IT-reliant healthcare system, using three real vignettes: an unconscious emergency room patient whose electronic health record (EHR) may have saved his life; a patient whose health problems and complex drug plan was effectively and holistically managed by pharmacists using a shared information system; and a bone marrow transplant patient who was able to remain in his remote community, and retain his quality of life, thanks to a Telehealth videoconferencing system. Discussion questions invite students to consider how IT plays a role in healthcare, the nature of “deep integration’, and why it takes so much time and money to develop effective IT for complex systems such as healthcare.
